Congenital Diaphragmatic Hernia Scholarship Fund

CDH
Scholarship Fund is for CDH survivors, siblings and
parents. Scholarships will be awarded based on
grades, extracuricualar activities and volunteer work.
Scholarship award decisions are made by the Advisory Committee.
We hope to award our first
scholarship for Fall 2010. An application form will be posted
soon.
It is our intention to fund these scholarships through donations and grants. Families can raise
money to give scholarships in honor / memory of their
cherubs.

Donations are tax-deductible.
You can donate in 1 of 4 ways:
1.
Through PayPal - a secure site that we've used for 10 years that takes
most major worldwide credit cards. Use the donation button

2. Through Firstgiving.com

secure site we've used for a few years. Simply

3. You can set up a
firstgiving page in honor or in memory of your cherub to raise money
for this fund as well.

If you would a Donation
Acknowledgment sent to the family of the person the donation was made
in
honor/memory of, please also send us their mailing address. The
Donation
Acknowledgment will only list your name(s), the name of the person(s)
the
donation was made in honor/memory of, and the date of the
donation.
We never list the amount of the donation.
